My 'Certificate of Live Birth' (State File # 57-321255, Local Registration District 3801, Certificate # 17591) has some missing information; the Place of Birth/Name of Hospital and the Address for that are not listed, (there is only a box checked next to 'inside city limits' and the city and county are listed as San Francisco), and neither of my adoptive parents signed or dated it confirming the information listed was correct.
My adopted mom was an RN at St Luke's in San Francisco, Ca. and I think I remember her telling me that's where I was born too.
My Attending Doctor: (the last name is hard to read so it may be incorrect) H. R. (Mclone or Mclove?), MD.
The Local Registrar: Ellis D Sox, MD. (His name was typed in the box where his signature should have been)
Date he received this document is listed as Nov. 20, 1957.
This is all the information I have. Both my adopted parents were only children and by the time I was born my fathers mother was the only relative they had still living. All 3 haved passed on now and there is no one else I know of to ask for help.
