The grand mother had a son also. Family name indicates Scotch ancestry. I was born 6-7 months weight 2 lbs.15 oz. on Nov. 14 1966 signed consent and waiver of parental rights. Had named me 'Terry Lynn' grandmother signed affidavit that mom had been living with her for two years. Discharged and placed in agency foster home Dec. 2 1966 and placed Dec. 20 1966 in home of Mr. and Mrs. Leonard A. Wiley. Adoption was finalized Sept. 19 1969. Birth certificate signed Sept. 6, 1966 two certificate numbers 66-030355 and 69-023222.
