I was adopted at birth. Born in Columbus Ohio (Franklin County) but adoption papers filed in Fairfield County (where my adoptive parents lived). My birth mother's maiden name is Doris Hannah, I believe she was married 3 times: #1- Jones: she signed 'Doris Ann Jones' on the adoption papers; #2- Loftis: on the 'Permanent Surrender of Child' form (filed on February 2, 1956) she was listed as Doris Ann Jones Loftis, and she signed the document as such; #3- Call: she married Edward B. Call in 1965+/-, they were granted a dissolution in 1978. She died in 1978. My birth father's name was not on my records, but his age was. My father may have been married.
