My siblings were abandoned at a babysitters in Colorado Springs. The last names and birthdates were aliases given by my father to the sitter. I have used the alias birthdate in filling out this form to keep down confusion when searching. The alias last name given to the sitter was Coultus. That is the birth name used for adoption. Another name given to the sitter for my brother was 'Boxer'. That was a nickname my father used for him.
